Case Manager III

General Description: 

• Under the supervision of the Child & Adolescent Services Director, Case Manager will obtain and coordinate services for individuals in Children’s Services, ensuring that the individual’s needs are met within the WRAP Net Work
• Work environment is such as to require sitting, walking, bending, and the full range of physical and mental activities associated with care giving
• Must be able to lift in excess of 45 pounds
• This position requires frequent, long-term contact with individuals, families and service providers
• Primary responsibilities include screening, intake assessments, crisis intervention, development of service plans and the implementation and monitoring of services for consumers

Salary:

• $15.37 - $17.31/hr (Depends on Experience)
